Accessibility Assistance Request and Report Form

Purpose

At the direction of executive sponsorship, in an effort to prepare for the DOJ accessibility compliance deadline of April 2027, an Accessibility Assistance Qualtrics form has been added in the footer of each agency website. Additionally, an option to request Accessibility Assistance will be added to the Feedback Tab.

What do you need to do?

No action is required from you. The DX team, with the support of Jessica Van Heuveln, Qualtrics Program Manager, has implemented these features onto the website footer and Feedback tab.

How will the requests work?

Submissions for Accessibility Assistance, whether from the footer or Feedback tab, will process into the same Qualtrics workflow to track and follow up. The DX team will receive and monitor these requests through June 2026. Starting in July 2026, the workflow will be directed to website agency managers. If there is another agency staff person that should receive these requests instead, please submit a Service Desk Plus ticket.
